Had a backload at 0200 with a PSV on DP in 25-knot wind; a tag line snapped and, as OIM, I called stop-work before the next lift. Operational integrity first — during SIMOPS, what gets your deck teams to use stop-work in the moment: radios on every spotter, a hard 2-minute pause before each lift, or something else that sticks?

What stuck for us on a 0200 DP backload was a physical red card — any spotter raises it and the crane freezes; we drill it until it’s muscle memory. Radios help, but in 25 knots people miss calls, so we pair the card with a headlamp sweep and a single word “freeze” everyone recognizes, and we brief that if a tag line snaps the next pick waits until spare lines/weak links are rigged. , this drives me nuts — do you run weak links on your tag lines or just beefier braid?
